A photograph inside a PDF is usually stored as an ordinary JPEG. Those come out byte for byte — the same file that went in, at full resolution, with nothing decoded or re-saved along the way. That is a very different thing from exporting the page as a picture, which gives you whatever the page happened to be rendered at.
Images stored as raw samples rather than a JPEG have to be rebuilt into PNGs. Still lossless, but no longer the original bytes.
A picture placed on twenty pages is stored once, so it comes out once. Anything smaller than about 80×80 is skipped — those are rules, bullets and spacers, not pictures. A few encodings, mainly JPEG 2000 and fax formats, have no safe way out; they are counted and reported rather than guessed at.

How is this different from PDF to Images?
PDF to Images renders each page as a picture, so you get whatever resolution the page was drawn at, background and all. This pulls out the pictures that were put into the document in the first place, at whatever size they were stored — which for a photograph is usually much larger.
Are the images changed at all?

Why did it find fewer images than I can see?

And a few encodings, mainly JPEG 2000 and fax formats, have no safe way out — those are counted and reported rather than guessed at.
Does it work on a scanned document?
Yes, and it is a good use for it. A scan is usually one large image per page, so you get the original scans back at full resolution.
